Abstract
Type 2 Diabetes Mellitus (T2DM) is a chronic metabolic disease that is defined by chronic hyperglycemia due to insulin resistance and progressive loss of insulin producing β-cells of the pancreas. Metformin is the first line of drug therapy but its effects on preventing β-cell deterioration are not that strong. Emblica officinalis (Amla) is a traditional medicinal plant with high antioxidant and antihyperglycemic activity that can potentially be used in diabetes management. The current study was designed to study the synergistic role of Amla and metformin against insulin resistance and pancreatic β-cell protection in experimental type 2 diabetes mellitus (T2DM). Wistar albino rats were used for experimental T2DM model induction using nicotinamide-streptozotocin (NA-STZ) model. Diabetic animals were randomly divided into six groups which included normal control (NC), diabetic control (DC), metformin treated (MT), Amla treated (AT), low dose combination (LDC), and high dose combination (HDC) groups (n = 6). This was done orally for 21 days. Therapeutic efficacy was estimated based on the changes observed in body weight, FBG, OGTT, fasting insulin levels, HOMA-IR, HbA1c, lipid profile, liver and kidney function parameters and histopathological examination of pancreatic tissue. Combination therapy had better antidiabetic action than monotherapy. Improvements in glycemic control, insulin sensitivity, serum insulin, HbA1c, lipid parameters, and organ function parameters were all significant. Histopathological analysis showed that the architecture of the pancreas was preserved and there was superior protection of β-cells in the high-dose combination group. The study demonstrates that Amla when given in combination with metformin enhances the therapeutic potential of metformin and can be a promising adjunctive therapy to enhance metabolic control and maintain the β-cell function in Type 2 Diabetes Mellitus.
