WhatsApp is the most widely used instant messaging platform in the world, and its low cost, near-universal familiarity, and multimedia capability have made it an increasingly common vehicle for delivering health education to patients with chronic, non-communicable diseases such as type 2 diabetes, hypertension, cardiovascular disease, and chronic respiratory illness. This review synthesizes the evidence on WhatsApp-based health education interventions for chronic disease management, drawing on randomized controlled trials, quasi-experimental studies, feasibility trials, and the broader mobile health (mHealth) literature. Across diabetes-focused trials, WhatsApp-delivered education has been associated with improvements in disease-specific knowledge, self-efficacy, self-care behaviors, and, in several but not all studies, statistically significant reductions in glycated hemoglobin (HbA1c). Evidence in hypertension and cardiovascular disease is comparatively sparse but suggestive of similar knowledge and adherence benefits. The review situates these findings within relevant behavior-change theory, including the Health Belief Model, Social Cognitive Theory, and the Chronic Care Model, and examines the platform-specific features, one-to-one messaging, broadcast lists, and group chats, that shape how WhatsApp interventions are designed and delivered. Cross-cutting methodological limitations are identified, including small sample sizes, short follow-up periods, inconsistent control-group conditions, and heterogeneous outcome measures, which limit the strength of causal inference that can currently be drawn. The review further examines an underexplored but consequential dimension of WhatsApp-based health education: the platform's data privacy and clinical-record-keeping limitations, which complicate its use for protected health information despite its pedagogical promise. The review concludes that WhatsApp-based health education is a feasible, acceptable, and probably modestly effective adjunct to standard chronic disease care, particularly in resource-constrained settings, but that the evidence base does not yet support it as a stand-alone replacement for structured self-management education, and that future trials should prioritize larger samples, longer follow-up, and standardized privacy safeguards.
