Abstract

ABSTRACT Background: Premenstrual syndrome (PMS) is one of the most prevalent gynecological conditions among women of reproductive age. A systematic review and meta-analysis found the pooled global prevalence of PMS to be approximately 47.8% (95% CI: 32.6–62.9%), with marked regional variation — the lowest at 12% in France and highest at 98% in Iran. Epidemiological data further suggest that 80–90% of women experience at least one PMS-related symptom, while approximately 2.5–3% meet criteria for the more severe premenstrual dysphoric disorder (PMDD). Despite this high burden, autonomic manifestations of PMS — particularly palpitations and excessive sweating — remain clinically underrecognized. Symptoms such as palpitations, dizziness, and sweating during the premenstrual phase are mechanistically linked to autonomic dysregulation, specifically hypersympathetic and hypoparasympathetic activity during the luteal phase. Concurrently, physical inactivity has emerged as a major public health concern. According to a recent Lancet analysis, nearly 49.4% of Indian adults fail to meet recommended physical activity levels, with projections indicating this figure may rise to 59.9% by 2030 if current trends persist. Globally, 32% of females above 18 years of age are physically inactive, with sedentary behavior increasingly linked to hormonal imbalance, autonomic dysfunction, and heightened stress responses. However, the specific relationship between sedentary lifestyle and premenstrual autonomic symptoms has not been adequately studied. Objectives: To determine the prevalence of palpitations and excessive sweating during the premenstrual phase among reproductive-age women, and to evaluate their association with sedentary lifestyle. Methods: A cross-sectional observational study was conducted among women aged 18–45 years with regular menstrual cycles. Women with known thyroid disorders, cardiac disease, pregnancy, lactation, or use of hormonal therapy were excluded. Data were collected via a structured, validated questionnaire comprising six sections: demographic profile, menstrual history, premenstrual symptom assessment (with severity grading), timing of symptoms, PMS screening using the Premenstrual Symptom Screening Tool (PSST), and physical activity assessment using the short-form International Physical Activity Questionnaire (IPAQ-SF). A minimum sample of 100 participants was targeted. Data were analyzed using SPSS. Descriptive statistics including frequencies, means, and percentages were calculated. The chi-square test was applied to assess the association between sedentary behavior and autonomic symptom occurrence, with p < 0.05 considered statistically significant. Logistic regression was applied where indicated. Expected Results: Evidence from cross-sectional studies suggests that over 53% of average-population women experience moderate to severe PMS symptoms, with sedentary individuals expected to show a higher burden of autonomic manifestations. A statistically significant positive association between physical inactivity and premenstrual palpitations and sweating is anticipated, consistent with existing evidence linking autonomic dysregulation to both sedentary behavior and the luteal hormonal milieu. Conclusion: Sedentary lifestyle represents a potentially modifiable risk factor for premenstrual autonomic symptoms. Establishing this association may improve clinical awareness, reduce misdiagnosis of these symptoms as cardiac or thyroid in origin, and support lifestyle-based preventive strategies in PMS management.
