Abstract
Background: Prehabilitation has emerged as an effective strategy to enhance postoperative outcomes in patients undergoing Total Knee Replacement (TKR). However, limited evidence is available regarding the combined use of cardiovascular and orthopaedic physiotherapy components within a single prehabilitation model. Aim: To evaluate the effectiveness of a hybrid cardio–orthopaedic prehabilitation physiotherapy program on functional capacity, pain, and postoperative recovery in individuals scheduled for TKR. Methods: A randomized controlled trial was conducted on 60 subjects awaiting TKR, randomly allocated into two groups. Group A received conventional prehabilitation consisting of strengthening, flexibility, and gait training. Group B received a hybrid program integrating aerobic training, quadriceps and hip strengthening, neuromuscular training, and endurance conditioning for 4 weeks. Outcome measures included the 6-Minute Walk Test (6MWT), Visual Analogue Scale (VAS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and quadriceps strength. Assessments were performed at baseline, pre-surgery, and at 6 weeks postoperatively. Results: Participants in Group B demonstrated significantly greater improvements in 6MWT distance (+22%), reduced pain scores (VAS), superior WOMAC functional outcomes, and earlier postoperative ambulation compared to Group A (p < 0.05). Quadriceps strength gains were also markedly higher in the hybrid intervention group. Conclusion: A hybrid cardio–orthopaedic prehabilitation physiotherapy program results in superior functional outcomes and enhanced postoperative recovery in TKR patients. Incorporating both cardiovascular conditioning and joint-specific rehabilitation into routine prehabilitation may improve surgical readiness and overall functional performance.
