Abstract
Type 2 diabetes mellitus (T2DM) is a significant global health concern. Guava, scientifically known as Psidium guajava L., is widely used in traditional medicine and cooking. Previous research has shown its anti-inflammatory and anti-diabetic properties, although the underlying mechanisms remain unclear. This study examined the potential therapeutic effects of guava leaf extract on T2DM using both animal and cell culture models. Over a 12-week period, diabetic mice were administered guava leaf extract orally. The results indicate improved insulin sensitivity and glucose tolerance, along with a significant decrease in fasting plasma glucose levels. Furthermore, the extract reduced the expression of genes involved in gluconeogenesis and promoted glycogen buildup in the liver, as well as enhanced glucose uptake. Moreover, treatment with GvAEx led to elevated expression of glucose transporter 2 (GLUT2) on the hepatocyte membrane. Additionally, we have elucidated, for the first time, the potential mechanism behind the hypoglycemic effect of GvAEx involving the intestinal microbiota. GvAEx induced significant changes in the gut microbiota composition, increasing the abundance of the Lachnospiraceae family and Akkermansia genus, which are known to produce short-chain fatty acids (SCFAs). These findings suggest that GvAEx may mitigate hyperglycemia and insulin resistance in T2DM by modulating liver glucose metabolism and restoring gut microbiota balance. Therefore, GvAEx shows promise for the development of therapeutics targeting T2DM.
