Epilepsy ranks among the most common neurological disorders, following closely behind stroke and dementia in terms of prevalence. This condition affects approximately 50 million individuals globally, with 90% residing in developing nations. Patients with epilepsy often endure significant emotional stress, behavioural issues, and intense social isolation. The underlying mechanisms that trigger seizures include neuronal hyperexcitability and a hyper-synchronous neural circuit. In Unani medicine, epilepsy is referred to as Ṣar‘or Mirgi, and is attributed to an imbalance in the body's humours, particularly Balgham (Phlegm) and Sawdā Black bile. The Unani approach to managing epilepsy is comprehensive, incorporating lifestyle changes, dietary advice and herbal treatments. The goal of these therapies is to rebalance the humors, strengthen the nervous system and reduce the frequency and severity of seizures. Commonly used herbal remedies include Āqarqarha (Anacyclus pyrethrum), Anīsūn (Pimpinella anisum) and Shankhpushpi (Convolvulus pluricaulis), which possess nervine and anti-spasmodic properties. This review aims to highlight the holistic approach of Unani medicine to epilepsy, covering its history, epidemiology, etiology, pathophysiology, classification, symptoms, diagnosis and management, as well as future trends in both Unani and conventional medicine.
