Abstract
ABSTRACT INTRODUCTION Benign breast diseases are the common problems in female of reproductive age which accounts for 90% of the clinical presentation. The common problems for which women are referred to surgery OPD are breast pain, palpable breast lump and nipple discharge. This causes much distress among the women who consider that every symptom in the breast is due to cancer which is consistent with the famous dictum of “Boyd”- “Every lump in the breast should be regarded as possible carcinoma until proved otherwise” However, sir Astley Cooper 1828 has shown the other side of coin. According to him “The disease of this organ has been too much considered as being malignant nature and women having lump in the breast have often been unnecessarily submitted to a surgery under the idea of being cancerous. AIMS AND OBJECTIVES To study the incidence of benign breast lumps in premenopausal women admitted in the department of surgery Rural hospital, Loni presenting as breast disease in the form of lump, nipple discharge and mastalgia. METHODS AND MATERIALS 1. STUDY TYPE: Observational Study 2. STUDY DESIGN: Descriptive Cross-sectional Study 3. STUDY SETTING: Department of General Surgery, DVVP Rural Hospital 4. STUDY POPULATION: Inclusion Criteria: Premenopausal women with breast lump of age group (15-45 years) Exclusion criteria: Post-menopausal women with diagnosed case of malignant breast lump, previous history of lumpectomy, mastectomy and oophorectomy 5. DURATION OF STUDY: 1 year (December 2022-December 2023) 6. Sample size: 60 7. Methods: Clinical Examination, Investigations (Routine Laboratory Blood tests, Ultrasonography, FNAC) RESULT • Maximum number of cases of benign breast lumps was found in the age group of 15-25 years (43.3%); fibroadenoma was found to be commonest in the age group of 15-25 years. • Fibroadenomas were found to be common in unmarried women while fibrocystic disease was found to be common in married women. • Fibrocystic diseases were found commonly in the age group of 26-35 years (38.3%), fibrocystic diseases were found to be common in married women (61.6%) • In 27.2% of cases of fibroadenoma and 55.5% of cases of fibrocystic diseases, pain was found to be exaggerated during menstruation. • FNAC showed overall accuracy rate in diagnosing fibroadenoma of 97.3% and an accuracy rate od 90.9% accuracy rate in diagnosing fibrocystic disease. • Accuracy of breast sonography in diagnosing fibroadenoma was 90.9% and in diagnosing fibrocystic disease was 66.6% CONCLUSION There was a good result after surgery of fibroadenoma, abscess, duct ectasia, duct papilloma and no recurrence found during the follow up period. Fibrocystic disease recurred in spite of treatment with drugs and assurance. As the period of study was one year, it is difficult to draw a conclusive opinion as the progress of benign lesion is very slow
