Abstract
Abstract Background: Labour and delivery are the shortest and most critical periods of the pregnancy-childbirth continuum because most maternal deaths arise from complications during delivery. The proportion of births attended by skilled health personnel is one of the key indicators for monitoring progress towards the attainment of the United Nations Sustainable Development Goals (SDG 3), which seeks to reduce the global maternal mortality ratio to less than 70 per 100,000 live births by the year 2030. This study investigated the determinants of skilled birth attendants use among women with children under one (1) year of age in the Nkwanta South Municipality of the Oti region, Ghana, using the Health Belief Model (HBM) as the conceptual framework. Methods: This is an analytical epidemiological study based on a cross-sectional survey design conducted in 2021. A sample size of 258 was used. Data was entered using SPSS version 20 and analyzed using both SPSS version 20 and STATA version 16. Descriptive statistics analysis was employed to determine outcome of interest in the sample. Background characteristics of respondents such as age, marital status, education, tribe, residence, occupation, religion, distance to health facility and four or more Antenatal Clinic (ANC) visits were all highly associated with births attended by skilled birth attendants (SBAs) (p<0.001). It was also found that in the HBM, strong association and high probability exist between perceptions on risk of delivery complications and none use of SBAs (aOR = 0.332; 95% CI = 0.141-0.780; p = 0.011). However, lack of knowledge on skilled birth was observed as one of the critical barriers to skilled birth attendant (SBA) use (aOR = 0.31; 95% CI [0.119-0.806]; p = 0.016). Conclusion: The proportion of births attended by skilled births attendants was high (77.5%). Nevertheless, 22.5% of women in the study area still patronize unskilled birth attendants. Concerted measures to improve utilization of SBAs should be targeted at the barriers to this intervention and strengthen its determinants.
