Introduction: Acharya Sushruta has very beautifully described the Asthmahagada Roga, one of among them is Ashmari (Nephrolithiasis). In the present era, it has become a global problem with a prevalence rate of 15% and a disease with a high recurrence rate due to food and lifestyle variations. Ashmari (renal stone) is one of the most common disease of Mutravaha srotas (urinary system) which occurs due to improper functioning of the filtration of the kidney gets formation of various types of crystals such as cystine, calcium, oxalate, and uric acid etc. Materials and methods: In this study, a 35-years-old patient attended the OPD on 11/02/2023 with a complaint of acute flank pain on the left side of the abdomen and back which was radiating from loin to the groin region, associated with indigestion, vomiting, burning micturition and dysuria. One month earlier, he had already a diagnosed case of renal calculi at the left upper ureter with dilation of the upper ureter and left renal pelvicalyceal system. In this present study, an attempt is made to study the effect of Aushadh like Trivikram ras, Varunadi kwatha, Syrup Cystone along with Sheeta Prabha vati. Result: There is a reduction in left renal calculi and the dilation of the left ureter is over resulting in normal size of the left ureter noticed after the USG done on 24/02/2023. Discussion: Here the filtration is affected due to the nucleation of crystals. Ayurvedic treatment including Shamana (pacification therapy) is adopted to reduce the size of renal calculi.
