Thyroxine had crystallised in 1914. Thyroxine was first synthesised as an acid in 1927, which restricted its oral absorption. In 1949, a sodium form of thyroxine was finally made available. After that, these artificial preparations were made available for medical application. Before 1970, LT4 and LT3 were typically replaced with extracts and combination therapy. However, endogenous production of T3 from LT4 in thyreotic patients was observed by peripheral deiodinase-mediated T4 to T3 conversion. For the majority of the 20th century, the market was controlled by natural thyroid preparations, such as thyroglobulin, desiccated thyroid, or thyroid extract, which include both triiodothyronine (T3) and thyroxine (T4). These preparations were the first pharmacologic treatments accessible. Although L-thyroxine monotherapy has been successful, some authors have questioned its effectiveness due to the fact that 10% to 15% of patients experience residual symptoms of hypothyroidism, which can include neurocognitive impairment.[3] and 15% of patients do not achieve normal serum triiodothyronine (T3) levels.[4]
