Abstract

Introduction : Scrub Typhus and Dengue are two of the commoner diseases in and around Pillayarkuppam in Tamil Nadu,Platelet deficit is seen in either but differential diagnoses depend on leukocyte count.To define Platelet crit and Platelet "mass resistant" was the aim and objective of this short communication. Methods : Screen capture of Leukocyte count and Platelet count was modulated in Scrub(leukocytosis) and Dengue(leukopenia) group of patients.Platelet transfusion in thrombocytopenia was warranted when platelets were below 40,000 cells/mm3.Platetet crit and Platelet mass wre calculated. Research : Platelet crit = Mean platelet count + (observed reticulocyte count - expected reticulocyte count) PCV-deposition of RBC’s to the Valsalva of the test tube with respect to platelet mass[(MCV/Platelet count*75)+2.5 to 4.25 being the normal value in adults weighing 70 to 75 kg]of such cases were low-“to the left” in normal leukocyte count.It was atypical to note that in Dengue high-“to the right” value for was found,mean value being 40%.With 40 as the reference range,it could be deduced that 4% alteration in PCV accounted for 1 Litre of “saline deficit” which was prevented with prompt therapy. The ‘platelet mass resistant’ that is the remainder of the value of the platelet crit is the volume of Fresh Frozen Plasma required to be replenished i.e For example,1.2 litres or 4 into 300 ml of yellow cold packets for platelets below 60,000.
