Abstract

Background: Although the risk in performing a shunt operation is low, the complications, such as mechanical malfunctions (obstruction of valve or catheter, catheter disconnection or migration), over drainage and infection are commonly encountered at St. Peter’s Hospital. The complications are frequently traced to ineffective identification and intervention of the various causes of shunt complications at different levels. When causes are not identified complications are inevitable, with subsequent care givers and surgeon’s dissatisfaction, lack of efficient use of scares resources like VPS. However, In Ethiopia there is no population studies analyzing the true incidence/prevalence of HCP and shunt complications. Objective: The aim of this study was to analyze the patterns, complications and the mortality of patients who underwent VPS insertion at St. Peter’s Hospital. Methods: A retrospective cross-sectional study on ventriculoperitoneal shunt outcome was conducted at St. Peter’s hospital. All patients diagnosed with hydrocephalus and have undergone ventriculoperitoneal shunt insertion from January 2016 - December 2020 GC at St. Peter’s Hospital and was on follow-up for at least 06 months postoperatively were included. Review was conducted from data collected from medical record numbers, and in person/telephone interviews using a standardized questionnaire developed by principal investigator. Results: From the 250 patients included in the study, 105 were female while the rest 145 were male. All of the patients were in the pediatric age group. 182 (72.8 %) of the sample population had communicative hydrocephalus; in almost half of the patients the cause of hydrocephalus was identified to be Post MMC repair HCP. The prevalence of complication among the study population was 26.4 %. Proximal catheter related complications were seen in 16 (24 %) of patients who have developed complications and out of these, 15 (6.0 %) had ventriculitis. Whereas, 57 (86 %) of patients with complications had distal catheter related complications and from these, 38 (66 %) had shunt infection. Positive correlations existed between the presence of a shunt complication and fever, vomiting, white blood cell count, CSF glucose, head circumference. Mortality was found to be 9.2% and 17.2% at 3 and 6 months post operatively. Convulsion, white blood cell count, CSF glucose and presence of complications were significantly associated with 3 and 6 month mortality rate. Conclusion and Recommendation: Prevalence of complications and the mortality rate was statistically comparable with other studies and infection was the most common complication aligning with findings from researches in developing countries. The findings of this study indicate that fever, vomiting, white blood cell count, CSF glucose, head circumference at presentation were independently significantly associated with the shunt complication, hence ,mortality at 3 and 6 months. Prospective controlled studies are required to address the observed associations between the risk factors and incidence of shunt complications and mortality Time frame: February 2022 to May 2022
