Abstract: ADPKD is a systemic disease, marked by a progressive increase of bilateral renal disease, diverticular disease., liver and pancreatic cystic disease, and ICA aneurysm in 6% of ADPKD patients. Other CNS manifestation of ADPKD is arachnoid cyst -8%, and chronic SDH. Most ADPKD with ICA aneurysm incidence is iceberg phenomenon due to its asymptomatic nature. In India, in my institute named omanthurar multispeciality hospital Chennai city., there is a case of 45 years aged male, with a known case of ADPKD., a patient coming with c/o headache. Patients taken for CT Angiogram for which have been revealed multiple aneurysms with varying sizes were located in Acom, Rt MCA bifurcation., and Pcom region. The Aortogram revealed an abdominal aneurysm too. This patient was taken up for a surgical procedure with all three aneurysms clipped in a single sitting surgical procedure.
