Abstract

Use of the qualitative methods was made to get detailed information using the emic perspective about the acceptability and willingness to use PrEP and rectal microbicides (RM) by men who have sex with men (MSM). These are new biological prevention tools for MSM who are at high risk for HIV. In-depth interviews with 39 consenting MSM were used to collect this information. “Expert peer” and social networking helped in recruiting respondents. The interview guide included a preamble about PrEP and RM to aid the discussion. Data collection and analysis occurred concurrently using a thematic analysis approach. The majority of the MSM were between 20-55 years of age, and mostly educated, 4 were HIV positive and 9 were in sex work. Many MSM perceived HIV risk despite using condoms due to the fear of condoms tearing/nonuse under the influence of liquor. All lacked knowledge about PrEP and RM. PrEP was acceptable because it provided added protection during condom tear/nonuse/multiple sex partners and for stress-free sex life. The majority of them were fine with daily pill intake until risk behavior lasts provided there were no severe and long-lasting side effects. Others, wanted it just before sex as sex was infrequent either available free/ at a cost below Rs 500/month. PrEP had to be absolutely efficient (100%) to be acceptable. All MSM were more willing to use RM for HIV protection as they were habituated to using lubricants as against PrEP preferably in gel formulation. Availability at a pharmacy with/without prescription; or NGOs/public hospital at nominal cost/free was preferred. Hence, awareness generation and making these prevention options available to the beneficiaries is urgently desired.
