Abstract: Epilepsy is the tendency to have unprovoked epileptic seizures. Anything causing structural or functional derangement of brain physiology may lead to seizures. The range of risk factors for the development of epilepsy varies with age and geographic location. The epilepsy is seen due to various viral, bacterial, fungal and parasitic infectious diseases of CNS, head trauma and tumors and can occur at any age, Congenital, developmental and genetic conditions are mostly associated with the development of epilepsy in childhood, adolescence and early adulthood. Brain infections caused by middle ear pathology like Cholesteatoma and middle ear tumors may also lead to epilepsy. Cholesteatoma may cause destruction of ear ossicles, erosion of bony labyrinth, canal of facial nerve, sinus plate of tegmen tympani and cranial wall and thus causes several complications like Extra duralabscess , Mastoiditis , Perisinus abscess , Petrositis ,Venous sinus , Facial nerve paralythrombosis , Otitic hydrocephalus , Brain abscess , Meningitis , Labyrinthitis , Subdural abscess which in turn lead to epilepsy and other life threatening conditions. By ignoring the ear examination we may miss the diagnosis of middle ear pathology which may spread the infection to CNS so we should not neglect the history of discharge from the ear, pain, vertigo...Etc and proper otoscopic examination should be done to diagnose in early stages. By diagnosing in early stages we can prevent the further complications so potential strategies are needed in early stage to prevent epilepsy resulting from brain infections and non-infectious inflammation caused by middle ear pathology.
